Body sculpting guide

How body sculpting actually works

An in-depth look at Accent Prime, VelaShape III, and Emsculpt — the technology behind each device, what a session feels like, how many you need, and what results to realistically expect at HBL Spa in Alexandria, VA.

Start here

Three different problems, three different machines.

Most people arrive asking for "body sculpting" when what they actually have is one of three distinct issues: a pocket of fat that will not move, skin and connective tissue that has lost its tension, or muscle that has gone soft. No single device solves all three, which is why the honest answer always starts with an assessment.

Localized fat generally responds to ultrasound and radiofrequency. The appearance of cellulite and skin texture responds to heat combined with mechanical manipulation. Muscle tone requires muscle stimulation. Below is how each of our devices works, what it feels like on the table, and a realistic view of what to expect.

The technology

Every device, explained in depth.

Accent Prime

Ultrasound + radiofrequency

How it works

Accent Prime (Alma Lasers) combines two energy types in one platform. Its ultrasound applicators direct acoustic energy into the fat layer, which the manufacturer describes as selectively targeting fat cells so their contents are released and then processed by the body's normal metabolic pathways. Radiofrequency applicators then heat the dermis and underlying connective tissue in a controlled therapeutic range, which is understood to contract existing collagen and stimulate new collagen production over the weeks that follow. In the US, Alma's Accent platform is FDA-cleared for non-invasive body contouring and for the temporary reduction in the appearance of cellulite.

What it feels like

The ultrasound phase feels like a firm, buzzing pressure, often with an audible high-pitched tone. The radiofrequency phase feels like a warm stone gliding across the skin — deeply warm, and it should not be painful. Your practitioner keeps the applicator moving and monitors skin temperature throughout.

Best for

Localized fat pockets on the abdomen, flanks, thighs, arms, and bra line — particularly when mild skin laxity is also a concern.

Session length
About 40–60 minutes per area
Recommended course
Commonly 6–10 sessions, spaced 1–2 weeks apart, depending on the area and your assessment
Results timeline
Some guests notice circumference or texture change partway through a series; collagen remodeling continues for weeks after the final session. Results vary by individual and are not guaranteed.
Downtime
Typically none. Mild pinkness and warmth for a few hours is common, and skin can feel slightly tender the next day.

VelaShape III

Infrared + bipolar RF + vacuum massage

How it works

VelaShape III (Candela) is FDA-cleared for the temporary reduction in the appearance of cellulite and for temporary circumferential reduction of treated areas. It combines infrared light, bipolar radiofrequency, vacuum, and mechanical massage: the light and RF warm the tissue and connective structures while the vacuum and rollers lift the skin toward the energy source and mobilize fluid. The combination of heat and mechanical manipulation is intended to improve the look of dimpling and the skin's surface texture.

What it feels like

A warm, deep suction-and-massage sensation — many guests compare it to an intense hot-stone massage. The vacuum level is adjustable, and temporary pink lines along the roller path are common.

Best for

The appearance of cellulite and circumference on thighs, glutes, abdomen, flanks, and upper arms; mild post-pregnancy laxity.

Session length
About 30–45 minutes per area
Recommended course
Candela's protocol for VelaShape III is typically 3 sessions about a week apart; some plans extend to 4–6 depending on the area and goal
Results timeline
Changes in texture and firmness generally build over the series and for several weeks afterward. Because the clearance is for temporary reduction, periodic maintenance is usually recommended. Results vary.
Downtime
Typically none. Temporary redness, warmth, or slight bruising in sensitive areas can occur and usually clears within a day or two.

Emsculpt

HIFEM — high-intensity focused electromagnetic energy

How it works

Emsculpt (BTL) is a muscle-stimulation device, not a fat-melting one. HIFEM energy passes through skin and fat to induce supramaximal muscle contractions that cannot be achieved voluntarily. A standard 30-minute abdominal session delivers thousands of contractions. Published studies — many of them manufacturer-sponsored — report increased muscle thickness and, in the abdomen, a reduction in the overlying fat layer. It is FDA-cleared for improvement of abdominal tone, strengthening of the abdominal muscles, and development of a firmer abdomen, along with clearances for the buttocks and certain other areas.

What it feels like

Strong, rhythmic, involuntary contractions that build in intensity — unusual at first, then generally well tolerated. Intensity is increased gradually to your comfort level, and lower-intensity tapping phases between contraction cycles help reduce post-session soreness.

Best for

Abdominal tone and core strength, glute lift, and rebuilding core engagement after pregnancy — with your physician's clearance if you have abdominal separation.

Session length
30 minutes, fully clothed, lying down
Recommended course
BTL's standard protocol is 4 sessions over about 2 weeks; maintenance sessions are optional
Results timeline
Many guests report feeling stronger soon after the series, with visible change typically developing over the weeks that follow. Results vary and depend on baseline fitness and consistency.
Downtime
Typically none. Mild muscle soreness for a day or so is common.

Lymphatic Drainage Massage

Manual technique

How it works

Energy-based sculpting creates fluid shifts and inflammatory byproducts in the treated tissue, and your lymphatic system moves that load out. Manual lymphatic drainage uses light, rhythmic, directional strokes along natural lymphatic pathways to support that process, which is why we build it into sculpting series rather than treating it as an afterthought.

What it feels like

Very light, slow, repetitive strokes — the gentlest treatment on our menu, and deeply relaxing.

Best for

Sculpting clients, general swelling and puffiness, and post-surgical recovery with your surgeon's clearance.

Session length
60 minutes
Recommended course
Paired with sculpting sessions, or weekly during a series
Results timeline
Many guests report reduced puffiness and a lighter feeling within a day. Lymphatic massage supports comfort and fluid movement; it is not a medical treatment and does not “detox” the body.
Downtime
None. Drink water afterward; increased urination for a day is normal.

What to expect

Your visit, step by step.

  1. Step 1

    Consultation and measurements

We assess fat versus laxity versus muscle tone, take baseline measurements and photos, review your health history, and build a device plan. Sculpting is not one machine for everyone — many plans combine two.

  2. Step 2

    Prep

You arrive hydrated and undress to the treatment area. Skin is cleansed, and a coupling gel is applied for ultrasound and radiofrequency devices. Emsculpt is done fully clothed.

  3. Step 3

    The treatment

Your practitioner works one zone at a time, monitoring skin temperature and your comfort throughout. You can talk, read, or rest the whole session.

  4. Step 4

    Drainage and aftercare

We finish with lymphatic work and go over aftercare: drink plenty of water, keep moving, go easy on alcohol, and avoid direct heat on treated skin for about 24 hours.

  5. Step 5

    Progress review

We re-measure and re-photograph as your series progresses so the plan is adjusted on what we actually see.

Am I a candidate?

Body sculpting is contouring — not weight loss.

  • You are reasonably close to your goal weight and at a stable weight
  • You have isolated, pinchable pockets rather than overall weight to lose
  • You want contouring, tone, or texture change — not weight loss
  • You can commit to a full series rather than a single session
  • You are not pregnant or nursing
  • You have no pacemaker or implanted electronic device, no metal implants in the treatment area, and no active infection there

Why HBL Spa

Devices are only half of a result.

The same machine in two different hands produces two very different outcomes. Applicator speed, tissue temperature, treatment mapping, and knowing when to stop are what separate a measurable change from an expensive warm massage — and that comes from over 20 years of practitioner experience.

We also refuse to sell single sessions as a solution. You get a mapped series, baseline measurements, lymphatic drainage built into the plan, and honest progress reviews along the way.

Before you book

A quick word on safety and expectations.

Non-invasive sculpting is well tolerated, but it is not for everyone. Let us know about pregnancy or nursing, a pacemaker or implanted electronic device, metal implants in the treatment area, active skin infection, hernia, blood-thinning medication, or recent surgery. If you are post-operative, we follow your surgeon's clearance before any device work begins. Results vary with body composition, hormones, hydration, and consistency — we will tell you plainly if sculpting is not the right tool for your goal.
